Fee of Applicable Category

Each initial application for a certificate of registration shall be accompanied by a non-refundable fee equal to the appropriate biennial fee. Fee payments by check or money order shall be made payable to Texas Department of State Health Services.

Multiple Categories of Use

In the case of a single certificate of registration that authorizes more than one category of use, the total fee is the category listed in 25 TAC, §289.204 with the highest corresponding fee. If the certificate of registration has additional authorized use locations, the registrant shall pay an additional 30% of the highest fee category for each additional use location.

Biennial Fees for X-Ray Registration

Biennial fees must be paid every two years in order to continue operating X-ray devices. The fee covers all direct and indirect costs associated with licensing a company or individual.

Amending a Certificate of Registration

To amend a medical or academic registration, submit RC 226-03.

Technical Renewal of Certificate of Registration

Renewal applications shall be submitted every eight years. The agency will send a renewal reminder letter and application by mail 45 days prior to the expiration date.
